Swallowing Evaluations and Treatment

Fiberoptic Endoscopic Evaluation of Swallowing (FEES) is a procedure used by speech-language pathologists (SLPs) to assess swallowing function. During FEES, a flexible endoscope is passed through the nose and positioned above the larynx to provide a clear view of the pharynx, larynx, and upper esophagus while the patient swallows.

FEES is performed to evaluate swallowing function, identify swallowing difficulties (dysphagia), and determine the cause of swallowing problems. It allows clinicians to directly observe the swallowing process, assess the coordination of the swallowing muscles, and identify any structural abnormalities or signs of aspiration (food or liquid entering the airway). 

FEES may be beneficial for individuals who have difficulty swallowing (dysphagia) or are at risk of aspiration due to conditions such as stroke, neurological disorders (e.g., Parkinson's disease, multiple sclerosis), head and neck cancer, traumatic brain injury, or other medical conditions affecting swallowing function. 

During the FEES procedure, a thin, flexible endoscope is inserted through the patient's nostril and advanced into the throat. The endoscope allows the clinician to visualize the structures involved in swallowing while the patient swallows various food and liquid consistencies. The procedure is typically performed in an outpatient setting and takes approximately 15 to 30 minutes to complete. 

Most patients tolerate FEES well and experience minimal discomfort during the procedure. Some individuals may experience temporary discomfort or a gag reflex when the endoscope is inserted through the nose, but this usually subsides quickly. Local anesthesia or numbing spray may be used to minimize discomfort during the procedure. 

FEES provides real-time visualization of the swallowing process, allowing clinicians to assess the movement of the swallowing muscles, the closure of the airway during swallowing, and the presence of any signs of aspiration (food or liquid entering the airway). It can help identify specific swallowing difficulties, such as difficulty with oral control, delayed swallow response, or impaired laryngeal function. 

The findings from the FEES procedure guide the development of individualized treatment plans for managing swallowing difficulties. Based on the assessment results, the clinician may recommend specific swallowing exercises, dietary modifications, compensatory strategies, or other interventions to improve swallowing safety and efficiency. 

FEES is considered a safe procedure with minimal risks. Potential risks include minor discomfort during the procedure, transient nasal irritation, or nosebleeds. Serious complications are rare but may include nasal mucosal injury, epistaxis (nosebleed), or bronchospasm (temporary narrowing of the airways).
